Inclusive Alliance Membership Inquiry Form

The Inclusive Alliance is accepting new members on a rolling basis. Prospective members must be approved by New York State to be a provider of services and support to individuals with intellectual/developmental disabilities, behavioral health issues, other health related concerns, youth and/or substance abuse services, or otherwise provides social determinants of health services. After completing this inquiry form, Inclusive Alliance will contact you to discuss membership requirements, the member fee, and approval process.
