Letters of Intent

Forward Leading IPA (FLIPA) and Inclusive Alliance IPA are proud to announce a collaboration for the Social Care Network RFA for the CNY region. As the lead entity, FLIPA is developing a SCN that leverages the strength and expertise of stakeholders from multiple sectors that serve the Medicaid population. Experience has taught us that real relationships are necessary to drive coordinated care. We hope you are willing to be one of our key partners on this important work.

As two locally led organizations deeply committed to our communities' health, FLIPA and Inclusive Alliance believe that our collaboration will significantly amplify our impact, driving positive, lasting change for those we serve. Together with you, we aim to create more accessible, efficient, and comprehensive care solutions. Learn more about the FLIPA as the SCN.

To share your support of this application and interest in being a key partner, please complete the following items and upload the documents. 

We will continue to accept Letters of Intent until April 4, 2024.


How To Submit Your Letter of Intent

Step 1. Download this template to use on your organization's letterhead. Please complete all information highlighted in red. Aligned with our mission that exclusive partnerships do not promote integration and better care for our communities, this letter does not affect your ability to also participate in other applications.

Step 2. (Optional) Download and complete all tabs in this CBO Data Collection file.

Step 3. Complete the form below and upload both documents.

- You can find your DOS ID here.

- If your organization would prefer to report your total annual budget instead of annual net income, please update the letter of intent accordingly.

-If you are not able to provide the data collection file, please still submit your  Letter of Intent.

Who can apply to be a SCN Lead?
501c3 nonprofits, including CBOs, IPAs, Health Homes, BHCCs, FQHCs, or PPSs with at least 3 years of experience working with CBOs in the application region.

As the lead entity, FLIPA is developing a SCN that leverages the strength and expertise of stakeholders from multiple sectors that serve the Medicaid population. Experience has taught us that real relationships are necessary to drive coordinated care. We hope you are willing to be one of our key partners on this important work.

FLIPA has developed relationships and
contracts across both the Finger Lakes and Central NY Regions that currently support key
aspects of the SCN; universal screening for social care needs, person-centered navigation
and connection to resources are part of the fabric of FLIPA. Streamlining of existing
network-wide standards of care, workflows, and processes allows more funding to flow to
the organizations in the communities who need it most and are doing the work of
supporting individuals. The proposal to support both regions also reduces the overhead for
operations, including resources for vendors, contractors, content expertise, and resources
for clinical support and supervision.

Do I need to complete the CBO Data File to submit a letter of intent?

The completion of this file is optional. You can submit your LOI without the data file. 

Can I report the total annual budget instead of annual net income?
Yes. If your organization would prefer to report your total annual budget instead of annual net income, please update the letter of intent accordingly.
