Network of 42 Nonprofits receives significant grant for local health initiatives
Syracuse, NY, January 9th, 2024- Inclusive Alliance, a leader in community-based health initiatives, is proud to announce its acceptance into the prestigious 2023-2024 Community Care Hub National Learning Community (NLC), organized by the Administration for Community Living (ACL). This groundbreaking program brings together 32 organizations from 23 states, aiming to foster shared learning and promote health-related social needs (HRSNs) through innovative community care hub models.
As a participant, Inclusive Alliance will play a crucial role in strengthening the bridge between community organizations, health care entities, and public health systems. This alignment is vital to reach and support underserved populations, addressing crucial HRSNs through coordinated services.
Separately, Inclusive Alliance has been awarded a generous grant of $180,000 from the Health Foundation for Western & Central New York. This funding is earmarked for building the technical infrastructure essential for a successful community care hub, underscoring the Foundation’s confidence in Inclusive Alliance’s capability to effect positive change in the health landscape of Central New York.

The NLC program launched in December 2023 and will continue through the summer of 2024, marking a period of learning & development for Inclusive Alliance.
